What is Embryo Biopsy?
Preimplantation Genetic Diagnosis is performed for screening the quality of an embryo at the genetic and chromosomal level. It is usually performed for patients who have the high risk of carrying an inheritable genetic disease. Making use of this method, embryo that are healthy and do not carry the disease are selected for transfer. Only a single cell from an 8-cell stage embryo is removed and sent for testing. A DNA analysis is performed for identifying embryos that are completely free from disease, thus making them apt for transfer.
Why is Embryonic Selection Advantageous?
A patient who has genetic disease, selection of embryos for implantation and develops into a healthy child becomes extremely crucial. Historically, the microscopic look of the embryo was the only criteria available to the patients and the doctor for selecting the healthy embryos for transfer. Unfortunately, it is important to know that there is no correlation between genetic health and embryonic appearance. In fact, even embryos appearing perfect might carry chromosomal abnormalities while unattractive embryos might be genetically normal. However, it was the advent of PGD that gave doctors and patients an effective way to identify genetic disease.
Clinical indications for using PGD include the following, but are not limited to:
- Aneuploidy testing for recurrent pregnancy loss (missing or duplicated chromosomes) and advanced maternal age
Embryo Biopsy for PGD and PGS
The procedure of IVF PGD and IVF PGS is usually performed on the embryos that are in developmental stages, thus the procedure for biopsy varies accordingly. There are two types of biopsy used in clinical practice:
Trophoectoderm Biopsy – On day five at blastocyst stage embryos, four or five cells are taken from the trophectoderm layer ensuring that there is no effecton the inner cell mass where the fetus develops.
Blastomere Biopsy – On day three at the cleavage stage embryos – one or two cells are removed from eight cell embryos to perform genetic analysis.
